Programs
Four programs, one promise: a family that comes to Larkspur has someone who knows them by name.
Healthy Start Home Visiting
Community health workers and doulas, most of whom grew up in the neighborhoods they serve, visit families at home from early pregnancy through twelve weeks after birth: about fourteen visits per family. Visits cover prenatal education, birth planning, postpartum recovery, breastfeeding and infant care. Every family is screened for depression, intimate partner violence, and food and housing insecurity at intake and at six weeks postpartum.
Who it serves: pregnant people in San Joaquin County, with priority for Medi-Cal members and first-time parents. About 300 families a year.
CalAIM Care Coordination
As a contracted provider with two Medi-Cal managed care plans, Larkspur delivers Enhanced Care Management for the birth-equity population of focus, plus Community Supports. A care coordinator helps members with appointments, prescriptions, transportation and benefits through pregnancy and the year after birth.
Bridge House
Six apartments of transitional housing for pregnant and parenting women who would otherwise give birth homeless. Residents stay up to 18 months with a case manager on site and an overnight resident aide. Most leave with a lease, a job or school plan, and a pediatrician.
Family Navigation & Lactation Hub
Opened September 2025 at our Stockton office. Walk in for help enrolling in Medi-Cal or WIC, a breast pump, or a session with a lactation specialist. Open Monday to Friday, 9 to 4.
Partners
Valley Oaks Medical Group, the San Joaquin Valley Doula Collective, two Medi-Cal managed care plans, WIC, and the county's perinatal equity program.
